I’ve always loved Cummins engines, and decided to buy a 2003 4BT from an old FedEx truck.

With what a new one costs, it generally wouldn't hurt to replace the engine, or rebuild it. If you are not experienced enough to undertake this endeavor, extract it and take to a reputable rebuilder.
